Registration for Shark Dive 2025

By signing the declaration to participate in this event, I acknowledge that the event has cost SIA £280 per person. I agree to the fundraising target specified below (excluding the registration fee and Gift Aid) and commit to raising sponsorship funds to meet or exceed that amount. I agree to send all sponsorship monies raised to SIA no later than four weeks after the event and will notify SIA as soon as possible if any circumstances arise that may affect my ability to meet the fundraising target, so we can discuss participation options. I also acknowledge that the event has a registration fee and that my registration will not be processed until this fee is received by SIA.

By signing this declaration to participate in this event, I acknowledge and understand that diving is an incredibly strenuous physical challenge, and that I am participating at my own risk. I agree to complete any medical forms deemed necessary by the Blue Planet diving team, and seek approval from a medical professional if needed.

I agree that by registering for this event, I am participating in an activity in support of SIA, who are managing certain elements of the event. However, the dive itself is facilitated by a third-party company, Blue Planet Aquarium, and I acknowledge that any liability related to the dive lies with Blue Planet Aquarium and not SIA. Only the data necessary for the activity will be shared with this third party, and they will not contact participants before, during, or after the event without express consent. By participating in this event on behalf of SIA, I also grant permission for SIA to use any photographs, videos, or testimonials taken during the event to raise funds or awareness for the charity across all media channels.
